Installer ignores txt.cfg content if burnt onto USB stick

I have prepared preseed ISO image following this tutorial: http://www.sgvulcan.com/2010/01/06/inst ... -only-ssh/
I have had to add only one entry related to keymap.
Next I test this image on virtual box and it worked as expected (it filled automatically all fields until SSH session became possible).
In the end I "burnt" ISO image onto USB stick. I have used many different options to do this (copy of iso content, rufus with different configurations, unetbootin and some command-line tools). In best case debian installer is loaded up but it absolutely ignores content of txt.cfg file (I cannot see custom label in menu, it doesn't install also).
Any ideas what can be wrong?
edit:
I forgot to add that there are two beeps when menu appears.
